ABD: Non painful, no masses palpated
U/G: no SS seen
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Assessment: open. concerned about underlying disease. fractured teeth

Prognosis:fair/guarded
Plan:cbc/chem/t4/chest rads
SURGERY: Permanent waiver due to illness
