KNOWN HISTORY: Whochoo was brought in as a stray, so there is no information on his behavior history or tendencies in a home environment. He was a little hesitant during intake and backed away initially, but he allowed all handling to be conducted. MEDICAL BEHAVIOR: Observed Behavior – easily handled, shy at first EVALUATION: Cage Condition: Cage is slightly re-arranged Reaction to assessor: Whochoo was lying down, calm and relaxed. Reaction when softly spoken to: Whochoo looks at the assessor with soft eyes and meows softly. Reaction to cage door opening: Whochoo becomes alert, but remains calm. Reaction to touch: Whochoo was a bit hesitant, but sniffs the assessor’s hand and accepts petting. He focuses on the assessor’s hand, but warms up and leans in for cheek rubs. He begins to purr and lifts his rear when pet on lower back, and then he shifts his body to face the assessor and comes forward to sniff and interact some more. Reaction to being picked up: Whochoo was tense during pick up, but allows handling. ACTIVITY LEVEL: Moderate VOCAL: Somewhat chatty CHARACTER TYPE: Shy, Sweet BEHAVIOR SUMMARY: Average Whochoo interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.

N History : stray Subjective: heavy ocular discharge OU, increased resp sounds Observed Behavior – easily handled, shy at first Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective T = 103.2 P = 120 R = 24 BCS 5/9 EENT: heavy ocular discharge and subpalpebral inflammation OU, nasal stertor Oral Exam: pink mm, healthy adult dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, increased bronchovesicular sounds and referred upper airway noise ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, skin free of parasites, multifocal alopecia along tail- no crusts or lesions CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: 1. URI 2. Alopcia of tail- wood’s lamp negative, r/o FAD 3. Conjunctivitis OU Plan: 1. DTM Fungal culture 2. Doxycycline 3. terramycin: both eyes twice daily for 10 days 4. Hold on Vx and SX pending improvement URI Prognosis: Fair SURGERY: Temporary waiver due to URI

Hx: was noted to be inappetant/lethargic in isolation yesterday, started cerenia and LRS SQ S/O QAR, resting in litter box and mewing constantly; regresses when approached appears to have a good appetite; has completely shredded bogus paper and cardboard feeding trays no nasal discharge or sneezing during observation OU-mild blepharospasms appears eupnic A URI inappetance/lethargy-appears resolved P CWCT, behavior did not allow cerenia/LRS treatment this morning but will try again tomorrow
