I came in the shelter as a STRAY on 11/25/2016 from NY 11433, owner surrender reason stated was STRAY.

microchip scan negative intact male, approximately 6 weeks old QAR, dehydrated, pink MM severe conjunctivitis OU – cleaned and applied terramycin very congested, moderate nasal discharge, sneezing URI and conjunctivitis confirmed by dr. 0844, start on: doxycycline 50mg/ml 0.2ml SID x 10 days, terramycin OU BID x 10 days, 10ml LRS SC SID x 3 days, nebulization SID x 3 days 1.5lbs; BCS: 3/5 has an okay appetite – will monitor gave 1ml nutrical, syringe fed kitten food, 10ml LRS SC and first dose of doxy friendly, allowed all handling

Appetite ok, eating ~50% Severe conjunctivitis OD – eye swollen shut with moderate hyperemia and mucoid ocular discharge OS moderately swollen but globe visible with moderate ocular discharge. 4-5 punctate lingual ulcers Moderate mucoid nasal discharge – audible nasal congestion and sneezing A: 7 week M DSH URI Conjunctivitis OU – not improving; suspect ulceration Lingual ulcers P: Continue doxycycline and terramycin in ISO Add ofloxacin OU BID x 10 days – reco NH transfer for more aggressive medical care and diagnostics Reco Fluoro stain + CXR Buprenorphine 0.3 mg/ml 0.04 ml OTM BID x 4 days Nebulize BID x 3 days Recheck 2 days
