I came in the shelter as a STRAY on 11/25/2016 from NY 11362, owner surrender reason stated was STRAY.

Medical rating was 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, behavior rating was NONE
microchip scan negative female, approximately 4 weeks old BARH ears are clean severe conjunctivitis OU – cleaned and applied terramycin as per dr. 0844 terramycin OU BID x 10 days no nasal discharge or sneezing observed 1.2lbs; BCS: 3/5 has an okay appetite – will monitor flea dirt on coat – treated with frontline spray friendly, allowed all handling
**** PLEASE NOTE **** KITTENS under the age of 8 weeks or weighing 2 pounds or less CAN ONLY be PULLED by a RESCUE!!!! IF INTERESTED, PLEASE contact a RESCUE ASAP!!!
