Medical Summary DVM Intake Exam Estimated age: 7-11 weeks based on dentition Microchip noted on Intake? Scan negative History : Stray Subjective: Calm, allows all handling Objective BCS 5/9, MMs pink and moist EENT: OU moderate mucoid discharge, chemosis, left eye initially stuck shut with discharge. Mild mucoid nasal discharge. Ears clean. Oral Exam: All deciduous teeth present. H/L: NSR, NMA, Referred upper resp sounds, slight tachypnea ABD: Non painful, no masses palpated U/G: Female kitten MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Conjunctivitis and URI Plan 1. LRS 30 ml SQ SID x5 days 2. Nebulize BID x5 days 3. Doxycycline 0.2 ml PO SID x10 days 4. Erythromycin OU BID x10 days 5. Monitor appetite Prognosis: Good SURGERY: Temporary waiver due to URI 1088
