Medical Summary Microchip: negative Sex: female intact Age- 4 weeks S-Found with severe injuries to front right and lef thind paw O-BAR, pink mm, CRT< 2 sec, hydrated EENT-CAU, COU, no nasal d/c PLN-all wnl H/L-no m/a, fsp, clear and eup GI-negative oral, SNP, NMP GU-FI, nsf MSI-BCS 3/5, amb x 4, distal digits RFL and LHL appear to be non-viable (cold to the touch and appears to have no blood flow, without sensation), RFL paw is swollen with open wound with bone visible, ok haircoat Neuro-alert and appropriate A-4 wo FI DSH 1. RFL and LHL distal digit wound/amputee P-recommend amputation of distal digits of RFL and LHL. Will be placed with NH prognosis fair to good with amputation
