Medical Summary 06/01/17 DVM Intake Exam Estimated age: 10 weeks Microchip noted on Intake? No. History : Stray kitten, no medical concerns noted. Subjective: Observed Behavior – Quiet, calm, allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. HR=220, RR=28, BCS=5/9. ORAL: All deciduous teeth erupted, excellent dental condition. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. ABD: Non painful, no masses/organomegaly palpated. U/G: Intact male, both testicles palpable. MS: Ambulatory x 4, with moderate left hind lameness. No palpable fractures. INTEG: Full, clean haircoat. No skin lesions or fleas. CNS: Mentation appropriate, no signs of neurologic abnormalities. A: Approx. 10 week old kitten with left hind lameness. DDx: soft tissue injury, fracture, other. P: Simbadol 0.1 ml SQ administered. Recheck exam tomorrow and x-rays if indicated. Good prognosis. Temporary waiver from neuter surgery due to lameness.
