SAFE 7/8/18 REN is a little kitten with a URI and conjunctivitis who was brought into the shelter. (STIMPY was euthanized due to humane reasons). Ren is emaciated and need lots of TLC and follow up vet care. Ren is friendly and would love to have a foster home asap.

History : Found outside with littermate. Overnight, pt was given 0.1 ml doxycycline and erythromycin ophth.
Subjective: Alert, mews, walks around kennel, eating on own
Observed Behavior – Allows all handling, seems to like petting
Evidence of Cruelty seen – None
Evidence of Trauma seen – None

BAR-H, MMs pink, BCS 3/9
EENT: Moderate conjunctival erythema, chemosis, mild serous discharge. Mild seromucoid nasal discharge. Ears clean.
Oral Exam: Clean teeth – all decid teeth erupted
PLN: No significant enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic, no coughing or sneezing
ABD: Soft, non painful, not distended, no masses palpated
U/G: Female intact kitten
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Normal externally
Assessment:
1. URI with ocular signs – moderate, suspect viral with secondary bacterial infection
2. Underweight
Prognosis: Good with appropriate treatment
Plan:
1. Doxycycline 0.15 ml PO SID x9 days
2. Gentamycin ophth drops OU BID; clean eyes as needed prior to medicating
3. Fortiflora SID x7 days
4. Vitamin B12 injection
