SAFE 7/8/18 MADOKA is a 6 week old kitten with a protopsed eye. Owner took kitten to vet but could not afford further treatment. Madoka also has a URI and conjunctivitis and needs a foster home where he can get further vet care and TLC.

Hx: Owner surrender. Conjunctivitis started 6/26 – client cleaned it but it got progressively worse. Client went to vet 7/2 and they recommended enucleation. Yesterday pt was given pen G and started on Clavamox and erythromycin ophth.
S: Alert, ambulates around kennel, allows all handling
O: Poor body condition. MMs pink.
-OD – ruptured globe, large amount of white cheesy discharge protruding from orbit
-OS – suspect ruptured globe; enophthalmos/microphthalmos, blepharospasm, corneal opacity, conjunctival erythema
-Mild serous nasal discharge
-AU moderate brown sandy discharge
-Eupnic, no coughing or sneezing
A:
1. URI
2. Suspect blind – ruptured and infected right eye, possible ruptured left eye (vs. congenital abnormality)
3. Underweight
4. Aural discharge – R/O otitis vs. ear mites
P:
1. Simbadol (given this morning) – 0.06 ml SQ SID x7 days
2. Vitamin B12 0.1 ml SQ
3. Fortiflora in food; guide pt to food bowl and encourage to eat
4. Clean ears q3 days x4 cleanings, instill Tresaderm AU BID x10 days
5. Discontinue Clavamox and start doxycycline 0.1 ml PO SID – mycoplasma is the most likely bacterial infection for feline URIs
6. Recommend enucleation OD +/- OS when pt gains weight and gets a little older

Madoka allowed counselor to pick her, when picked up she meows and then starts purring, when placed on the table she explores her surroundings and will try to walk away. Madoka allowed counselor to collar her and place her into a carrier.
