SAFE 10/14/16 **Patch was nervous and bit a staff member, so is in on a DOH bite hold for 10 days, but has a possible leg fracture, so could use follow up care at a vet / and hold at a vet facility for the holding period. Won’t you help her today?**

Medical Behavior Evaluation BLUE
Medical Summary 10/2/16 Check injured kitten. Microchip scan neg. O: QAR. Fearful, hissing in carrier. Able to examine when restrained. EENT: No oculonasal discharge. ABD/UG: Palpation WNL. Female. MS: Amb x 3. Non-weight bearing left front. Multiple fractures palpable left humerus and leg is dangling from body. Two small degloving wounds over antebrachium and carpus. These wounds are dry, no bleeding. Distal limb is cool with no sensation. Integ: Left lat thorax – open wound with skin hanging down ventrally exposing subcutis, muscle fascia. No penetration into thoracic cavity. A: Multiple fractures L. fore with loss of use of the limb. Wound l. lat thorax. P: Amoxi/Clav 45.7 mg/ml 0.25 ml PO given; continue BID. Cleaned wound with chlorhexidene. Start Buprenorphine 0.5 mg/ml 0.06 ml SQ BID (if kitten too fractious, can apply PO). Good prognosis with appropriate care, will need amputation of left front leg. Rec. placement – may need to pursue before DOH hold is completed. Pyrantel 0.2 ml PO given. Applied Activyl, gave FVRCP.

A: FRACTURE AND NECROSIS OF LF LIMB; WOUND AT L. THORACIC REGION; R. INGUINAL HERNIA
P: CONTINUE WITH CURRENT TREATMENT AND MONITORING PLAN. REC PLACEMENT ASAP AS ANIMAL IS LIKELY TO BECOME SEPTIC WITHOUT LF AMPUTATION. FAIR PROGNOSIS WITH APPROPRIATE TREATMENT AND CARE

Recheck fractured leg and wound
O: QARH. Hissing. mm=lt pink, moist.
MS: Left fore – limb distal to fracture site at humerus cold, with necrotic areas developing. Non-weight bearing.
Integ: Ventral abdominal bruising and mild swelling. No palpable body wall hernia.
Wound L. lat thorax – mild purulent discharge.
A: Severe fracture with limb necrosis. Wound. Soft tissue injury on abdomen.
P: Will need surgical treatment of limb soon or kitten is at risk for worsening condition and possible need for EHR. Fair to good prognosis.
