I am a female black domestic sh mix. The shelter thinks I am about 6 MONTHS old.
I came in the shelter as a STRAY on 11/27/2016 from NY 11004, owner surrender reason stated was STRAY.

11/28/2016 Exam Type INITIAL – Medical Rating is 3 NC – MAJOR CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 4.1 LBS.
Scanned negative *Unable to collar due to temperament QARH – hangs back, flees, when cornered tries to scratch, may bite. Exam begun in towel, concluded in net. F~ 6 months Ears, eyes, nose, throat clear Teeth white. Gums slightly pale Coat clean – no flea dirt or parasites noted Ambx4 Nails long – unable to trim **During exam, began to urinate. Urine dipstick on voided sample showed 3+blood USG =1.030 3+Leukocytes Vet rx 0.18cc Convenia, recheck in 5, 10, 14 days.

11/30/2016 BEHAVIOR EVALUATION – NH ONLY
Exam Type BEHAVIOR
Mustard was brought in as a stray, so we cannot speak to her behavior in her previous home. Reaction to assessor: Mustard was resting on her blanket near the front of the kennel. Reaction when softly spoken to: Mustard keeps her tail at her side, glances over at the assessor and doesn’t get up or come forward when coaxed. Reaction to cage door opening: Mustard flinches and remains motionless. Reaction to touch: Mustard lip with wide eyes focused on the assessor, looks around for an exit then retreats to the back. Her movements are quick, she shifts away from contact with her body and tail lowered then quickly rushes forward attempting to flee. Behavior Determination: New Hope Only Mustard is displaying behaviors that preclude placement in the adoptions room and may require further investigation before placement in a home. She is extremely fearful in the shelter environment and does not currently tolerate petting or handling. The behavior department feels that placement with a New Hope Partner is the best option at this time.
