Medical Summary DVM Intake Exam Estimated age: 2 yr Microchip noted on Intake none History : stray with bloody diarrhea Subjective: qar moderate dehydration Observed Behavior – growls but allows handling Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BAR decreased skin turgor P = wnl R = wnl BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pink, tacky PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: scrotal testes MSI: Ambulatory x 4, skin free of parasites, no masses noted, decreased skin turgor, blood stained hindlimbs patch of alopecia and scabbed wound left metacarpal region CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment dehydration r/o HX hematochezia Plan lrs 50ml SC sid x 3 days monitor for relapse of diarrhea parvo snap test Prognosis: fair SURGERY: Temporary waiver due to dehydration
