GAIL – GRAY KITTEN:
DVM Intake Exam
Estimated age: 4 weeks
Microchip noted on Intake? no
History: Found stray, no medical history.
Subjective: Quiet, unkempt, significantly bulging eye OD
Observed Behavior: quiet but allows all handling
Evidence of Cruelty seen: none
Evidence of Trauma seen: none
Objective
BCS 4/9
EENT: OS WNL with no discharge or conjunctivitis; significant conjunctivitis with bulging eye OD. Unable to visualize cornea to determine whether eye is proptosed or viable.
Oral Exam: all deciduous teeth present, no visible ulcers
PLN: No enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic
ABD: Non painful, no masses palpated, moderately distended intestines
U/G: female, no spay scar
MSI: Ambulatory x 4, skin free of parasites, no masses noted, diffusely unkempt hair coat; Woods lamp negative
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: severe URI and conjunctivitis OD. May require enucleation in the future; unable to determine until conjunctivitis has resolved somewhat.
Plan: Start erythromycin OU BID x 10 days and doxycycline 0.1 mL PO SID x 10 days. Seek NewHope foster for home care and possible surgical intervention.
Prognosis:
SURGERY: Temporary waiver due to age, severe URI

Microchip noted on Intake? no
History: Found stray in yard, no medical history
Subjective: QAR, euhydrated, significantly smaller and unthriftier than sibling in same carrier.
Observed Behavior: nice, allowed all handling.
Evidence of Cruelty seen – none
Evidence of Trauma seen – none
Objective
BCS 3/9
EENT: Ears clean, no nasal discharge noted. Marked conjunctivitis OU. Cornea visualized OD – cloudy with synblepharon. Too much pus present OS to visualize cornea.
Oral Exam: all deciduous teeth present, no oral ulcers.
PLN: No enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic
ABD: Markedly distended and full of firm stool.
U/G: intact male, could not palpate both testicles (too small)
MSI: Ambulatory x 4, skin free of parasites, no masses noted, diffusely unkempt but Woods lamp negative
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: severe conjunctivitis OU with URI. Unable to determine whether cat will be visual after conjunctivitis resolves; likely to have severe deficits OU. May require enucleation OS; cannot determine whether eye is intact under pus.
Plan: Gave warm water enema and 0.5 mL lactulose for constipation. Start erythromycin ointment OU BID x 10 days and doxycycline 0.1 mL PO SID x 10 days. Seek New Hope foster with potential enucleation required/potentially blind at end of disease course.
Prognosis: good for life; open for vision
