06/19/17 13:31 Barack is an unaltered female DSH mix; she came to her owner’s home as a gift an is being ssurrendered due to the owner having no time. Sh has no known health issues or injures an has no recent vet history. Around strangers Barack is friendly and tries rub up on them. Barack has been around children and is usally friendly and gentle with the children. Barack has never been around other cats and has no bite history. Barack only likes to be held for short period of time, and will scratch and struggle out of hold. She goes into the carrier easily and was descibed as mellow, friendly and affectionate with a medium activity level. She is feed friskies dry food and is litter boxed trained with an uncoverd box with clumping litter. She has a horizontal scratching post made of cardboard. Upon intake Barack was friendly and allowed the counselor to collar and take photos of her with no issue.

KNOWN HISTORY: Previously lived with: Adults and spent time with children Behavior toward strangers: Friendly and outgoing Behavior toward children: Friendly and gently Behavior toward cats: Unknown Behavior toward dogs: Unknown Bite or Scratch history: None Litter box training: Yes Energy level/descriptors: Mellow, friendly and affectionate with a medium activity level Other notes: She only likes to be held for short period of time, and will scratch and struggle out of hold MEDICAL BEHAVIOR: Very nice cat but complains with in-depth handling ie. for blood draws ENRICHMENT NOTES: 06/20/17 Comes forward when spoken to and vocalizes. Allows petting along her head and body while leaning in and raising tail, making eye contact with soft eyes. So cute and sweet! Pretty senior lady, doing well after first day! EVALUATION: Cage Condition: No change Reaction to assessor: Barack comes forward with a relaxed body and greets the assessor. Reaction when softly spoken to: Barack solicits attention and gently rubs her head against the kennel door. Reaction to cage door opening: Barack is calm and relaxed. Reaction to touch: Barack nuzzles up against the assessor’s hand and leans in for attention. She gives soft head butts and enjoys petting and attention all over her body. Reaction to being picked up: Barack was a bit tense when held and tried to wiggle out of the assessor’s arms. ACTIVITY LEVEL: Moderate VOCAL: Quiet CHARACTER TYPE: Easy going, Affectionate, Social BEHAVIOR SUMMARY: Beginner Barack interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. No known history of behavioral problems. This cat can go to a beginner home.

DVM Intake Exam Estimated age: 10 years – cannot assess dentition because no teeth present Microchip noted on Intake? no History: Owner surrender, no previous medical history or known issues Subjective: BAR, euhydrated, shedding copiously Observed Behavior: very nice cat but complains with in-depth handling ie. for blood draws Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 7/9 EENT: Eyes clear, ears clean, no nasal discharge noted; old aural hematoma AS Oral Exam: no teeth noted in any arcade PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female; did not shave for spay scar MSI: Ambulatory x 4, skin free of parasites, no masses noted, multiple mats on the dorsum. Mats shaved – skin underneath intact with no excoriations. Two small scabs over the right scapula with no discharge or evidence of infection. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: healthy geriatric cat with no remaining teeth Plan: ok to move to adoptions; still needs spay scar check Prognosis: excellent SURGERY: Permanent waiver due to geriatric
