10/27/2016 INITIAL PHYSICAL EXAM
Medical rating was 4 C – SEVERE CONDITIONS , behavior rating was NONE
Sex: Male Scan: Negative Estimated age: 6-8 weeks based on weight and dentition Fleas: Some flea dirt. S: Hissed repeatedly and tried to climb up the wall of the cage when I approached, but once I touched his head and picked him up, he did not try to bite, scratch or escape. O: BAR-H, BCS 3/9, MMs pink EENT: Large amount of crusted discharge in fur under eyes and on eyelids. Severe chemosis and conjunctival injection. Corneas appear clear but difficult to fully assess due to severe chemosis. Serous nasal discharge. Tartar on deciduous teeth. H/L: NSR, NMA. Upper airway congestion. Quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. Moderate amount of flea dirt. No skin lesions noted. UG: Male intact kitten A: 1. URI and severe conjunctivitis 2. Underweight Short-term prognosis: Good P: 1. Administered 0.2 ml pyrantel PO and Frontline spray during exam. 2. Erythromycin OU BID x10 days 3. Doxycycline 50 mg/ml 0.15 ml PO SID x10 days 4. Nebulize BID x5 days 5. Monitor appetite 1088
