Medical Summary DVM Intake Exam Estimated age: ~5 years Microchip noted on Intake? placed on intake History : owner passed away Subjective: QAR Observed Behavior – tense/nervous but allows handling Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 9/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk, sl tacky; CRT <2 sec PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: soft, nonpainful; deep palpation difficult due to BCS but no obvious abnormalities palpated U/G: female; possible spay scar palpated on abdomen MSI: reluctant to walk; dandruff, unthrifty coat along dorsum; smells strongly of urine with mild urine staining on abdomen CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Severe obesity Unthrifty coat-likely due to undergrooming secondary to BCS Plan Monitor overall well being, if any concerns about appetite or other systemic signs, then rec’d CBC/chem Rec’d significant weight loss through dietary management Prognosis: good SURGERY: suspect spayed but if not, would give temp waiver d/t obesity
