9/18/16 SND, sneezing seen on rounds. Move to isolation, start doxycycline 1 ml PO SID x10 days. 1088 —- 09/08/16 18:04 s/o BAR oligodontia miliary dermatitis, probably related to flea infestation shy but solicits scratching. heart and lungs wnl a: generaly healthy, some dental disease and flea dermatitis p: treat skin with convenia injection 0.45ml remaining teeth will probably fall out on their own but dental would be ideal good prognosis 9/7 Microchip: negative Sex: intact female Age: appx 14y Mentation: QARH Eyes: clear Ears: waxy but WNL Nose: no d/c Teeth: missing all but upper left canine and wear on other teeth, heavy staining, swollen gums (do not appear to be extractions, possible roots retained) If abnormal BCS: WNL, muscle wasted Skin: some flea bite dermatitis along scapuls Hair Coat: dull and live fleas seen Declawed: no Any injuries: no Behavior: nervous, allowed all handling, eager to go back into carrier Medication: N/A, preventatives given

9/7/16 19:52 Basic information: Chuck was brought in as a stray. The finder found Chuck in a building hallway, the finder placed Chuck in a cat carrier and brought Chuck to the shelter. No other information could be provided during intake. Intake Behavior: During intake Chuck had tense, stiff body. Sydney would retreat from extended hand. Chuck was not willing to allow himself to be scanned collared, pictured, and picked up/held, Chuck showed signs of fear towards the handler.

Reaction to door opening: Chuck stands with tail up, relaxed body posture, soliciting attention. Reaction to touch: Chuck leans into the assessor’s hand, rolls over, and appreciates petting on the head and body. Reaction to Being Picked up: She becomes antsy then jumps back into the cage. Placement determination: Average Chuck interacts with the observer, appreciates attention, is easy to handle and tolerates all petting.
