DVM Intake Exam Estimated age: ~8 years Microchip noted on Intake? negative History : stray Subjective: QAR, ~5-6% dehydrated. No csvd. Eating well. Observed Behavior – very sweet. Easily handleable but prefers less restraint. Got irritated and tried to get away and whipped head when restrained and for oral exam. Did well for all medical handling and procedures Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = eupneic BCS 4/9 EENT: Eyes clear, ears have moderate brown waxy debris, no nasal discharge noted Oral Exam: moderate dental disease with dried crust around mouth, no oral lesions noted PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MN MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric-possibly just dehydrated vs chronic condition Dental disease FIV+ Plan: Continue to monitor while at BACC Start LRS 100ml SQ SID x3d Recheck tomorrow Clean ears Prognosis: Fair SURGERY: neutered

Progress exam History : stray Subjective: QARH. No csvd. Eating well (2/3 meal eaten). Likes dry food. U in box but no BM. Overall is stable. Objective P = wnl R = eupneic BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: moderate dental disease with dried crust around mouth, no oral lesions noted PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MN MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric Dental disease FIV+ Plan: Continue to monitor while at BACC Continue LRS 100ml SQ SID until 2/2 Recheck tomorrow +/- move out of medical Rec NH placement > SCP due to condition Prognosis: Fair

Recheck dehydration S/O: BAR. ~5% dehydration. Allows all handling, attention seeking. Eating well. Small amount of normal urine and normal stool in litterbox EENT: Nuclear sclerosis, iris atrophy, no ocular or nasal discharge, moderate dental staining, mild gingivitis, ears WNL HL: Normal thoracic auscultation ABD: Soft, non tender INTEG: WNL MS: Ambulatory x 4 UG: Neutered A: Geriatric, est age 10-12yrs, FIV+ Mild dehydration persists despite eating well and supportive care P: Blood work and UA pending. Continue to monitor while at BACC

CBC Mild monocytosis – all other values WNL CHEM Mild hyperglobulinemia (5/4g/dL) Moderate hyperthyroidism (11.8ug/dL) All other values WNL A: Geriatric, FIV+, hyperthryoidism P: Rec methimazole 2.5mg PO q24 x 14 days. Recheck blood work in two weeks to recheck values. Good short term prognosis with appropriate treatment

No csvd. Good appetite. U+ BM-. Comfortably resting in the cage and purring. Doing well. KNOWN HISTORY:: Atlas was brought in as a stray, so we cannot speak to his behavior in his previous home.
MEDICAL BEHAVIOR:: 01/31/18 very sweet. Easily handle able but prefers less restraint. Got irritated and tried to get away and whipped head when restrained and for oral exam. Did well for all medical handling and procedures
Cage Condition:: Cage is neat
Reaction to assessor:: Atlas was relaxed and lying down on his blanket near the back of the kennel.
Reaction when softly spoken to:: Atlas makes eye contact and meows softly, but he doesn’t get up or come forward when coaxed.
Reaction to cage door opening:: Atlas remains calm.
Reaction to touch:: Atlas leans forward to sniff the assessor and accepts gentle petting on his head and along his body. He rises up and comes forward, arches his back and gives head butts when pet.
Reaction to being picked up:: Atlas was calm, relaxed and allowed all handling.
