Medical Summary 04/05/17 History – Stray kitten. Microchip scan neg. Subjective Observed Behavior – Friendly, social, allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – Yes. Objective: BARH. mm=pink, moist, CRT<2 s. P = 200 R = 24 BCS = 5/9 ORAL: All adult teeth erupted, no oral lesions. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: NSR, NMA. Lungs clear, eupneic. ABD: Non painful, no masses palpated. U/G: Female, unspayed. MSI: Ambulatory x 4, toe-touching lameness left hind. Swollen left hind digit 5 and distal metatarsals 4-5, painful on palpation. Wound with mild serosanguineous discharge lat digit 5. Scabbing in fur on ventral thorax. NEURO: WNL. A: Lamness and wound left hind. R/o soft tissue injury, fracture. Otherwise apparently healthy. P: House in medical area. Buprenorphine 0.3 mg/ml 0.2 ml SQ administered. Continue BID x 5 days. Convenia 80 mg/ml 0.22 ml SQ administered. Will need sedation and xrays of left hind paw. Good prognosis.
