Medical Summary DVM Intake Exam Estimated age: 5-10 years based on dentition and overall appearance Microchip noted on Intake? Scanned negative History : Stray, no hx given Subjective: Allowed some exam but struggled to get away from oral exam and close restraint. Once in kennel, hissed whenever I approached. Objective BCS 3/9, MMs pink, estimated 8% dehydrated EENT: Mild brown crusty dried discharge at medial canthus OU. Ears clean, no nasal discharge noted Oral Exam: Mod-severe tartar, right maxillary canine tooth is partially extruded and the crown is chipped PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male intact, testicles s/s MSI: Diarrhea caked all over hind legs and tail. Moderate flea dirt. Ambulatory x 4, no masses noted. CNS: mentation appropriate – no signs of neurologic abnormalities Parvo snap test – negative Assessment 1. Diarrhea, dehydration- R/O GI parasites vs. bacterial enteritis vs. other cause 2. Dental disease 3. Flea dirt 4. Emaciation Plan 1. Deworm 2. CBC/chemistry and FeLV/FIV test 3. Metronidazole 50 mg/ml 0.5 ml BID x7 days 4. Monitor diarrhea 5. LRS 150 ml SQ SID x5 days 6. Recommend dental cleaning and rads with extractions as needed Prognosis: Fair SURGERY: Temporary waiver due to DIARRHEA 1088
