Medical Summary DVM Intake Exam Estimated age: 7yrs Microchip noted on Intake? none History : o died Subjective: when first brought in this AM was open mouth breathing, then calmed down and became tachypneic Observed Behavior – allows all handling, gentle Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = 200 R = 60 BCS 6/9 EENT: Eyes clear, cross eyed, ears clean, mild serous nasal discharge noted, menace intact OU, mydriatic pupils, but PLR’s intact OU Oral Exam: pink gums, moderate focal tartar and mild gingivitis in caudodorsal dental arcades PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, tachypneic, but no open mouth breathing ABD: Non painful, no masses palpated U/G: FS- palpable spay scar, mammary glands very small, no palpable MGTs MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment dental and periodontal dz overweight tachypnea: r/o stress, secondary to congestion, other Plan cbc/chem in house as general health screen: mild neutrophilia (stress), mild hyperglycemia (stress), mild hyperglobulinemia (r/o normal for p, occult infectious/inflammation)- overall, bloodwork is NSF monitor app long term rec dental px with possible extractions, long term rec approx 1-2 lbs weight gain Prognosis: good SURGERY: spayed already N/A
