Victory 13113 – 4 month old male white w/ brown tabby, stray:
Behavior Notes:
Upon intake Victory was timid and was placed in a feral den.
Medical:

Progress exam
History : Stray intake 11/19-had mild symptoms of URI with swelling under eyes with pitting edema. Had fractured upper left canine tooth.
Subjective: QARH. No c/s/v/d. Facial swelling seems to be improved. Not interested in food but very nervous. No bm or u in litter box.

P = WNL
R = WNL
BCS 4/5
EENT: Eyes clear no d/c, ears clean, no nasal discharge, dried blood on dorsal rostral aspect of nose, very mild swelling under left eye but most of facial swelling has resolved
Oral Exam: no oral ulcers, kitten dentition
PLN: No enlargements noted
H/L: NSR, NMA, Harsh BV sounds bilaterally
ABD: Non painful, no masses palpated
U/G: MI
MSI: Ambulatory x 4, no masses noted, very dusty/dirty haircoat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Assessment:
Facial swelling- r/o trauma vs. other-improving
URI
Nasal discharge r/o secondary to facial swelling vs. URI-resolved
Dirty haircoat r/o smoke inhalation causing URI
Inappetence
Prognosis: Fair to good
Plan:
Start doxycycline 10mg/kg PO SID x10d until 11/30
Recheck daily-facial swelling +/- URI +/- smoke inhalation
If still not eating tomorrow consider cerenia, LRS, and mirtzapine
If lung sounds are not improving consider BW and CXR

ABD: Non painful, no masses palpated

MSI: Ambulatory x 4, fleas and lost of flea dirt, no masses noted, very dusty/dirty haircoat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal:
Assessment: Facial swelling- r/o trauma vs. other
Nasal discharge r/o secondary to facial swelling vs. URI
Prognosis: Fair to good

Recheck for URI signs and facial swelling tomorrow, consider ABs vs. anti-inflammatories vs. other. May need sedated skull rads vs. other diagnostics in the future
