Medical Summary 04/04/17 History – Brought in by owner’s landlord due to a fire at the home. No known medical concerns. Microchip scan neg. Subjective Observed Behavior – Social, friendly, active. Allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. P = 220 R = 28 BCS = 5/9. ORAL: All adult teeth erupted. No dental lesions. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: Lungs clear, no dyspnea, no murmurs/arrhythmias. ABD: Non painful, no masses palpated. U/G: Female, no spay scar found. MS: Ambulatory x 4, no lameness. INTEG: Old flea collar on neck too tightly – removed. Fur in this area is worn, but no underlying skin irritation. Otherwise full, clean haircoat, no wounds or skin lesions. CNS: mentation appropriate – no signs of neurologic abnormalities. A: Apparently healthy young adult, age est 1 year. P: Monitor for any respiratory problems that may suggest irritation from smoke. OK for OVH. Excellent prognosis.
