DVM Intake Exam Estimated age: 4 years Microchip noted on Intake? no History : surrender Subjective: patient was hissing, lunging and persistently swatting at the carrier while attempting to get patient out of carrier Observed Behavior – unable to handle with out sedation Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective P = 174 R = 34 BCS 7/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: wnl PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: nueterd male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment – Plan sedate with Telazol 0.25ml IM Prognosis: good
