History: Patient did not vomited this morning. No coughing or sneezing. Obj EENT- Mucous nasal discharge. Skin- No ectoparasite seen. Unkempt hair coat. Abdomen- NAF Ms- Normal muscle mas Behavior BAR Neuro- Ambulatory in all 4 limbs CBC- THROMBOCYTOPENIA Chem 17- Mild BUN elevation TT4 elevated Plan Treatment for hyperthyroidism is indicated Recommendation: 2.5 mg methimazole SID PO for 2 weeks . Then readjust dose.

06/22/17 16:17 Basic information Smokey is a 16 year old female white and gray DSH with no known medical conditions. Smokey was surrender to ACC along with her house mate because of the owns health and moving. Socialization and behavior Smokey has not lived with children and behavior is not known for interacting with them.. Smokey has lived with other cats and is affectionate and playful with them. Smokey played gently with the other cat. Smokey s has also lived with dogs and is respectful around them.. Around strangers is shy for a few hours . When playing with her owner Smokey s played gently. Smokey s does not have a scratch or bite history. Smokey was never given a bath or had her nails trimmed. Smokey enjoys being brushed. Smokey isn’t bothered by being picked up and held but will jump out of your arms after a few minutes. For the new family to know Smokey was described as talkative and playful with a medium activity level. when home with Smokey she liked to be in the same room as people. Smokey enjoyed playing with tinfoil balls. Smokey s is litter box trained and used a covered litter box with Fresh step clumping litter. P Smokey ate fancy feast wet food and Friskies dry food. Smokey did not have a scratching post at home and liked to scratch on the wood molding. Behavior during intake Smokey was talkative during intake. Smokey allowed to be scanned and collared but was a little shy. Smokey allowed all handling.

Other notes: Smokey liked to be in the same room as people and enjoyed playing with tinfoil balls. MEDICAL BEHAVIOR: Very nervous but allowed the whole exam. EVALUATION: Cage Condition: Cage is neat Reaction to assessor: Smokey was lying flat in her litter box, alert. Reaction when softly spoken to: Smokey shifts her body slightly, but keeps a low body posture. Reaction to cage door opening: Smokey picks her head up and looks around, curious. Reaction to touch: Smokey sniffs the assessor’s hand and accepts petting. She warms up, leans in for rubs, and begins to purr. She approaches the front after and leans against the assessor. Reaction to being picked up: Smokey remains calm. ACTIVITY LEVEL: Moderate VOCAL: Quiet CHARACTER TYPE: Shy initially, Sweet, Easy going BEHAVIOR SUMMARY: Average Smokey interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. No known history of behavioral problems. This cat is showing behavior appropriate for new or experienced cat parents.
