History : new intake on 4/21/17 Subjective: Observed Behavior – calm, friendly, eating a little wet food on exam (after cleaning face) Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T =— P = WNL R = WNL BCS 4/9 EENT: crusted MP nasal discharge, dried ocular discharge, ears clean Oral Exam: deciduous dentition, no lingual ulcers seen, mm’s pink PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, no alopecia seen, moderate dehydration CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment ~5 week old MI DSH kitten URI intact dehydrated Plan Doxycycline (50 mg/ml): 0.15 ml PO SID x 14 days Nutrical 2 ml PO BID x 5 days LRS 12 ml SQ SID x 2 days Erythromycin OU BID x 7 days Prognosis: Good SURGERY: Temporary waiver due to underage/underweight and URI
