Medical Summary DVM Intake Exam Estimated age: 4-5 weeks Microchip noted on Intake? neg History : brought in with 2 other kittens; observed to be eating on their own; diarrhea staining on all of them Subjective: BAR Observed Behavior – interactive, allows handling Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T =not taken P =WNL R =WNL BCS 3/9 EENT: Eyes clear, ears clean, no nasal discharge noted; food material on face Oral Exam: mm pk, moist; CRT <2 sec; deciduous premolars coming in PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: pot bellied appearance; soft, nonpainful with no palpable abnormalities; diarrhea staining on fur U/G: male MSI: Ambulatory x 4, skin free of parasites, no masses noted, coat stained with food and diarrhea CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment 1. diarrhea-r/o panleukopenia vs parasites vs naive immune system Plan parvo test littermate monitor diarrhea-if not improving or becomes lethargic/inappetant then add metronidazole needs foster Prognosis: excellent SURGERY: Temporary waiver due to age/weight
