Hello, my name is Sabrina. My animal id is #8656. I am a female white cat at the Manhattan Animal Care Center. The shelter thinks i am about 2 years old.
I came into the shelter as a stray on 02-Oct-2017.

[DVM Intake] Estimated age:2yrs Microchip noted on Intake? none History :found as stray Subjective:weak, allows all handling, not interested in food Observed Behavior – no aggression Evidence of Cruelty seen -no Evidence of Trauma seen -no Objective P = unable to ascult cardiac sounds R = purring, but eupneic BCS= 3/9 EENT: moderate purulent nasal d/c, occluding nares, dried d/c all over face, no ocular d/c no squint Oral Exam: moderate dental and periodontal dz, unable to fully open mouth to assess PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FI, grossly normal MSI: unkempt furcoat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment 5% dehydrated URI congestion questionable appetite underweight Prognosis: good if responds Plan: LRS 50ml SQ (first dose given 7PM) SID x3 move to medical iso nebulization BID x5 days cerenia 10mg/ml: 0.13ml SQ SID x3 doxycycline 50mg/ml: 0.3ml PO SID x10days mirtazapine 15mg: 1/4T PO once SURGERY: Okay for surgery Temporary waiver due to Permanent waiver due to
Hx: came in with severe URI; doxycycline was recommended but never started; was on cerenia, SQF and mirtazapine for 3 days S/O -QAR, very affectionate; allows all handling -seems interested in food but when she approaches bowl, she chatters teeth as if nauseas -mm pk, tacky; CRT <2 sec; moderate tartar with mild discoloration of upper canines; gingival bleeding along incisors but unable to identify wound or source of bleeding; no oral ulcers noted -severe mucoid nasal discharge, congestion; fur along front limbs and face stained with dried mucoid discharge -purring loudly on auscultation -soft, nonpainful abdomen -diffuse muscle wasting A URI-severe, has not been receiving treatment P doxycycline 0.7 ml PO SID x 10 days cerenia 0.4 ml SQ SID x 4 days LRS 100 ml SQ SID x 4 days nebulization BID x 5 days prognosis: guarded to fair; will hopefully resolve once antibiotic therapy started
Recheck for severe URI: S/O: Quiet, responsive, mildly depressed due to illness – responds well/perks up when stimulated, friendly behavior, purring during exam very affectionate; allows all handling EENT: heavy purulent nasal discharge, large lingual ulcer on tip of tongue; MM pink,tacky, CRT ~ 2 sec H/L nma, mild cough – amb x 4, BCS emaciated/muscle wasting – soft abd – female A: severe URI, lingual ulcer, poor BCS, dehydrated – appears stable at this time and responding to tx P: CWCT, monitor appetite – Simbadol 0.5 ml SQ SID x 5 days for ulcer Px: remains open => guarded to fair, with response to tx
Recheck for severe URI: S/O: Quiet, responsive when examined, purrs and seeks attention though mildly depressed due to illness, rests a lot in cage – condition appears to be improving mildly, eating better with less nasal discharge and nose cleaned well today – body condition remains poor, SQ LRS fluids palp. on ventral chest A: severe URI with lingual ulcer, poor BCS – appears stable at this time and responding to tx P: CWCT, monitor appetite Px: remains open => fair, with response to tx

Behavior Assessment Date: 10/6/2017 Retest Date: Retest Reason: Next Test Date: KNOWN HISTORY: Sabrina was brought in as a stray, so we don’t have any behavioral history or tendencies in a home environment. ENRICHMENT NOTES: Seems extremely rundown and sickly. Resting on bedding, eyes half-open, soft posture. Lay in place when door opened. Allowed all pets, purred. Fell back asleep after door closed. Cage Condition: Cage is neat Reaction to assessor: Sabrina remains neutral, lying down on her cage bedding during the approach. Reaction when softly spoken to: Sabrina lifts up head, soft eyes, ears erect and forward. Reaction to cage door opening: Sabrina remains immobile in place. Reaction to touch: Sabrina allows the touch, purrs in place and appreciates the petting on the head and body. Reaction to being picked up: Unable to perform do to medical condition. ACTIVITY LEVEL: Laid back VOCAL: Quiet CHARACTER TYPE: Calm Sweet POTENTIAL CHALLENGES: Other Potential challenges comments: Please note that Sabrina is being treated for an upper respiratory infection at the time of the behavior assessment. This condition may affect some of the behaviors shown during evaluation. BEHAVIOR DETERMINATION: Average Behavior Asilomar H – Healthy BEHAVIOR SUMMARY: Sabrina interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.
