Oral Exam:moderate staining and tartar, gingivitis, nose- clear
Heart: wnl
Lungs: wnl
Abdomen: wnl
Musculoskeletal: spotted undefined mild skin lesion seen back and neck- possible flea allergy dermatitis- wood lamp- negative, vet is requested to check
Mentation: no abnormal neuro sign seen
Preliminary Assessment: flea allergic dermatitis, gingivitis
Plan: needs dvm exam
