DVM Intake Exam
Estimated age: 1yr
Microchip noted on Intake? no
History: owner surrender with 3 neonate kittens
Subjective: BAR
Observed Behavior – head pressing, allows all handling
Evidence of Cruelty seen – no
Evidence of Trauma seen – no
P = 200 R = 24 BCS 5/9
EENT: Eyes clear, AU – dark debris, no nasal discharge noted
Oral Exam: scant tartar
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: intact female, mild blood in fur around vulva, recent partuition – brought in with 5 day old kittens
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, but soaked in urine
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: apparently healthy post-partuition dam
Plan: medical monitor, SQF 75ml
Prognosis: good
SURGERY: Temporary waiver due to nursing neonatal kittens

Basic information
Lilo is a 1 year old DSh. Owner has had Lilo for 1 year and is surrendering her along with her 3 babies because of fleas.
Socialization
Lilo has lived with children ages 9 and up and is relaxed and affectionate with them. Lilo plays gently with children and adults. Around strangers Lilo has lived with other cats and does not fight with them but will chase them and eat their food. Lilo has not lived with dogs. Lilo has never bitten with the owner but was very stressed in the center and bite( did not break skin).
Behavior
In car rides Lilo will meow. Lilo has never been given a bath by previous owner before. Lilo is ok with trimming her nails at first but after a few minutes she becomes afraid and will try to get away. Lilo enjoys being brushed and isnt bothered when being picked up and held.
For the new family to know
Lilo was described as affectionate and mellow with a low activity level. She enjoys playing with balls and when home with her she will follow you around. Lilo is litter box trained and uses a covered litter box with clumping litter. Lilo is mostly kept indoors but has gone outside a few times. Lilo eats Dry food mostly but has eaten people food. When its time for bed Lilo likes to sleep in the bed with the owner. Lilo does not scratch on anything ion the house.
behavior during intake
Lilo was very stressed during intake. Lilo allowed handling at first but became more agitated and would try to flee. Lilo bite the counselors hand after being placed in a carrier but let go right away. Lilo allowed limited handling.
