Oral Exam: mild-mod tartar and gingivitis
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Female, intact – no spay scar found. Umbilical hernia, approx 2cm and non-reducible
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: externally normal
Assessment: Dental disease, umbilical hernia
Prognosis: Good with hernia repair and dental cleaning
Plan: Recommend spay and hernia repair. Recommend dental procedure.
