Medical Summary 06/04/17 DVM Intake Exam Estimated age: 1 year Microchip noted on Intake? No. History : Found as a stray, no medical concerns noted. Subjective: Observed Behavior – Very fearful, initially allowed handling when wrapped in a towel. Became increasingly agitated and began hissing, growling, and striking. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. HR=200, RR=20. BCS=4/9. ORAL: All adult teeth erupted, no oral lesions. EENT: No oculonasal discharge, no sneezing. H/L: Lungs clear, no murmurs/arrhythmias. PLN: No enlargements noted ABD: Non painful, no masses/organomegaly palpated. U/G: Female, no obvious spay scar palpable, unable to shave. MSI: Ambulatory x 4, no lameness. Full, clean haircoat. Skin free of parasites, no masses noted. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Apparently healthy approx. 1 yr old cat. P: OK for OVH surgery. Excellent prognosis.
