Medical Summary 06/04/17 DVM Intake Exam Estimated age: 4 yrs Microchip noted on Intake? Yes #985112008181967 History : Brought in by owner, no medical concerns noted but he has a history of aggression per owner and has bitten people in the past. Subjective: Observed Behavior – Fearful, growling and hissing, strikes out when approached. Limited exam due to behavior. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist. HR=220, RR=20. BCS=5/9. ORAL: No obvious oral or dental lesions. Full oral exam not performed. EENT: No oculonasal discharge, no sneezing. Ears not examined. H/L: Lungs clear, no murmurs/arrhythmias ausculted. PLN: No enlargements noted ABD: Very tense and reactive when abdomen is touched. No obvious abnormalities palpable but unable to palpate deeply. U/G: Neutered male. MS: Ambulatory x 4, no lameness. INTEG: Full, clean haircoat. No obvious skin lesions. CNS: Mentation appropriate – no signs of neurologic abnormalities but full neuro exam not performed.. A: Apparently healthy. P: Monitor while at BACC. Excellent prognosis.
