Medical Summary DVM Intake Exam Findings History: Stray found under a car Subjective Observed Behavior – very friendly, head bumps, rolls on side Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = 200 R = sniff BCS 3/5 (4-5/9) EENT: Eyes – mild blepharospasm, unable to assess whether primary or secondary from scratching at ears; large amount dark brown waxy debris AU, very pruritic; mild serous nasal discharge noted; sneezed once Oral Exam: healthy adult dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: bladder large but soft MSI: Ambulatory x 4, small amount of flea dirt, no masses noted, hair coat dirty CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Otitis externa, AU; possible early URI signs (monitor) Medical Behavior Status GREEN Medical Status 2NC Asilomar Status TREATREHAB Fast Track?
