Medical Summary DVM Intake Exam Findings History: Stray found under a car Subjective Observed Behavior – very friendly, head bumps, rolls on side Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = 200 R = sniff BCS 3/5 (4-5/9) EENT: Eyes – mild blepharospasm, unable to assess whether primary or secondary from scratching at ears; large amount dark brown waxy debris AU, very pruritic; mild serous nasal discharge noted; sneezed once Oral Exam: healthy adult dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: bladder large but soft MSI: Ambulatory x 4, small amount of flea dirt, no masses noted, hair coat dirty CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Otitis externa, AU; possible early URI signs (monitor) Medical Behavior Status GREEN Medical Status 2NC Asilomar Status TREATREHAB Fast Track? No Plan Schedule for neuter. Acceptable candidate for adoption or rescue pending behavior evaluation. Cleaned AU and applied Tresaderm – continue BID x 7d, then recheck. Monitor for URI.
