Medical Summary DVM Intake Exam Findings History Subjective -this cat appear older than she is due to dental dz–changed age to 4 Observed Behavior – calm, allows handling Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective P = 160 R =wnl BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: oligodontia. remaining teeth have advanced dental disease PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, advanced pregnancy, at leat two feti palpated U/G: intact, pregnant MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment dental dz. pregnant Plan OHE this cat will naturally lose her teeth eventually, but a dental would make her much more comfortable good prognosis
