Medical Summary DVM Intake Exam Estimated age: 10-12mos Microchip noted on Intake? no History : Found as stray in Citi Field with two other cats. Subjective: Cat is BARH. Observed Behavior – Cat is squished against back of feral den, pupils dilated (fearful). Looks like she wants to flee. Hissed, lunged, and swatted at assess-a-hand. Sedated with 0.1 mLs Telazol IM, which provided adequate sedation for exam and treatments without stressing cat out. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = wnl R = wnl BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Healthy adult dentition; missing 101 and 201 PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: wnl; bladder large, but soft MSI: Ambulatory x 4, live fleas, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Apparently healthy Plan: Acceptable candidate for adoption or rescue pending behavior evaluation. Okay to schedule spay. Prognosis: Good-Excellent SURGERY: Okay for surgery
