History : Stray found on someone’s back porch, unknown history, laterally recumbant
Subjective:
Observed Behavior –
Evidence of Cruelty seen – No
Evidence of Trauma seen – Possibly yes
Objective
T = 100.5
P = 220bpm
R = 40bpm
BCS 2.5/5
EENT: Menace present, PLR abnormal, pupils dilate and constrict minimally when light applied, dazzle present, ears clean, no nasal or ocular discharge noted
Oral Exam: Clean teeth, adult dentition
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: F
MSI: Non-ambulatory,unable to hold head up, CP deficits x4, withdrawal present in both front legs, intermittent withdrawal present in hind legs, skin free of parasites, no masses noted, healthy hair coat, no abrasions or wounds seen
CNS: Mentation inappropriate, intermittently seems to be unaware of surrounding but occasionally tracks with eyes.
Rectal: Anal tone present
Assessment:
Suspect head trauma
Prognosis: Guarded
Plan:
Full body x-rays- no obvious fractures or abnorms present
IVC
IVF bolus (33mL over 20 mins)
Mannitol 1g/kg IV over 20 mins
Fluids @ 2x maintenance
Lasix 1mg/kg IV
Monitor response to treatment- if improves will continue supportive care, if not improving consider EHR
Postpone vaccines for now
