Oral Exam: missing canine 104, mild/mod. calculus/dental dz
PLN: No enlargements noted
H/L: elevated HR, NMA, CRT < 2; Lungs clear, eupnic
ABD: soft, resists palp. of cranial abd., no masses palpated
U/G: abdominal/spay scar palp. and observed through thin abd. skin
MSI: poor BCS with signs of general muscle wasting/chronic weight loss, mod. dehydrated, Ambulatory x 4, long nails, thinning skin, no external parasites observed, no masses noted, unkempt coat
CNS: mildly depressed though appropriate – no signs of neurologic abnormalities
Rectal: externally normal
Assessment: geriatric cat with signs of underlying chronic health condition
+ poor BCS, dehydrated
Prognosis: open
Plan: monitor in medical for next 24 hrs
+ CBC/Chem/T4/UA
+ 150 ml LRS SQ
