History : TNR cat. Caretaker noted it to be drooling and limping this morning, was normal yesterday. Gave it an unknown dose of buprenex orally.
Subjective: QAR. <5% dehydrated. No csvd.
Observed Behavior – Very nervous, tries to flee, but able to do exam with restraint
Evidence of Cruelty seen – no
Evidence of Trauma seen – no
Objective
P = 180bpm
R = 40bpm
BCS 5/9
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: Mild to moderate ptyalism, mild to moderate dental tartar/periodontal disease
PLN: mildly prominent popliteal LNs, mildly prominent submandibular LN
H/L: NSR, NMA, CRT <2; mild tachypnea, mildly harsh lung sounds bilaterally
ABD: Non painful, no masses palpated
U/G: neutered male, no discharge
MSI: Ambulatory x 4 with intermittent LFL lameness- no overt fractures palpated, skin free of parasites, no masses noted, few old scabs noted on skin, scab on nasal planum, expressed small amount purulent discharge
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: normal externally
Two view thoracic radiographs- Mild to moderate diffuse bronchial pattern noted, no overt pulmonary infiltrates noted; cardiac silhouette appears wnl
Two view left forelimb radiographs- No overt fractures identified
CBC: Mild to moderate leukocytosis (24.91), Moderate neutrophilia (20.84), mild monocytosis (0.78)
Chem: Hyperglycemia (302), mild hyperglobulinemia (5.5)
T4: wnl
Assessment
1) FIV+
2) Mild Lymphadenopathy
3) Mild tachypnea/harsh lung sounds- based on radiographs, no overt evidence of pneumonia, heart failure or neoplasia. Based on lung pattern, could potentially be mild allergic bronchitis (asthma).
4) Abscess
5) Hyperglycemia- r/o stress vs metabolic (diabetes)
6) Ptyalism- r/o dental disease vs nausea secondary to systemic disease
Prognosis: fair to guarded
Plan:
Gave Convenia 8 mg/kg SQ once
Start LRS 100mls SQ SID x 3 days
Start Maropitant 1 mg/kg SQ SID x 3 days
Collect urine for UA (put nosorb in cage)
Given Butorphanol 0.4 mg/kg IM once on intake
Start Onsior 2 mg/kg SQ SID x 3 days
Start Simbadol tonight 0.24 mg/kg SQ SID x 5 days
Start gabapentin 20mg/kg PO BID (for anxiety) x 7 days
CTM in medical- monitor respiratory rate closely as well as appetite
