I am a male black and white domestic sh mix. The shelter thinks I am about 8 YEARS old.
I came in the shelter as a STRAY on 11/12/2016 from NY 11221, owner surrender reason stated was STRAY. I came in with Group/Litter #K16-081290.

11/15/2016 Exam Type RE-EXAM – Medical Rating is 3 C – MAJOR CONDITIONS , Behavior Rating is NONE, Weight 6.1 LBS.
11/15/16 Monitor appetite, condition. O: BARH. Social, excellent appetite. Grooming himself. EENT: No oculonasal discharge. INTEG: Rostral upper lip swelling and ulceration. No bleeding, mild moisture. Small amount of dried material in fur around mouth. A: Improving overall, appears comfortable. P: Continue current therapy. Good prognosis. 11/14/16 Recheck appetite, overall condition. O: BARH. Active, social. Eating canned food very well, drinking well. ORAL: No obvious changes in oral ulcers. Subjectively seems less painful today. EENT: No oculonasal discharge. INTEG: Rostral lip swelling/ulceration more pronounced on left side. Mild crusting around this area and on face. PLN: Mod. enlargement of mandibular lymph nodes. A: Stable condition. P: Continue current therapy. Fair to good prognosis. 11/13/16 Check swollen upper lip, oral ulcers. O: QAR. mm=pink, moist, CRT<2 s. All food eaten overnight. ORAL: Several ulcerated areas on rostral tongue, hard palate. Sensitive on exam. Mod. calculus, missing teeth, oral odor. EENT: No oculonasal discharge. Wax buildup AU (diagnosed with ear mites yesterday). H/L: Lungs clear, no murmurs. ABD: Palpation WNL. INTEG: Dry, unkempt haircoat. Debris dried in fur over paws. Rostral upper lip – mod swelling, ulceration, discharge. MS: Amb x 4. Slightly thin body condition. A: Age est. 8 years. Oral ulcers – R/o viral infection (calici), eosinophilic plaques. Rostral lip swelling and ulcer – R/o eosinophilic plaque, infectious disease. Dental disease. P: Start antibiotics and pain medication: Rx AmoxiClav 45.7 mg/ml 1 ml PO BID x 10 days. Buprenorphine S/R 0.11 SQ q 72 hours. Monitor appetite, general condition while at BACC. Fair prognosis.

11/12/16 19:39 BASIC INFO Buffalo A1096682 and Amityville A1096683 are dsh cats that were found in finder’s back yard. Finder placed in a carrier and brought them to ACCNYC. BEHAVIOR DURING INTAKE Buffalo and Amityville allowed all handling, collaring and photos.

Amityville was brought in as a stray, so we cannot speak to his behavior in his previous home. Reaction to assessor: Amityville comes forward when the assessor approaches then gently brushes up against the kennel door. Reaction when softly spoken to: Amityville is very sweet and reaches out with paws towards the assessor. Reaction to cage door opening: Amityville is calm and relaxed. Reaction to touch: Amityville purrs softly then gently brushes up against the assessor for attention. He allows petting all over, is very affectionate and appreciates attention. His body is relaxed and he rolls over on his side when rubbed on his cheeks. Reaction to being picked up: Amityville is calm, relaxed and has a soft body when held. Behavior Determination: Beginner Amityville interacts with the assessor, solicits attention, is easy to handle and tolerates all petting. No known history of behavioral problems. This cat can go to a beginner home.
