Medical Summary DVM Intake Exam Findings History Subjective Observed Behavior – friendly, allows most handling, but when given food allows almost all handling, no agression Evidence of Cruelty seen – underweight- memo states found in home with other underweight cats, some were dead, o was taken to hospital for possible illness Evidence of Trauma seen – Objective T = not taken P = 182 R =eating food ravenously (otherwise didn’t want to sit still), eupneic, but unable to obtain RR BCS 3/9 EENT: Eyes clear, ears clean, no nasal discharge noted, approx 3-5% dehydrated Oral Exam: pink gums PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI, both testicles descended MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: underweight, mild dehydration Fast Track? no Plan LRS 50ml SQ SID x4, gave first dose today at intake too thin to be neutered at this time- rec gain more weight first. Recheck weight in one week then re assess- temporary waiver for neuter until appropriate weight prognosis: good
