Medical Summary DVM Intake Exam Estimated age: ~7 years Microchip noted on Intake? positive 985112006122584 History : stray, was in house fire reportedly Subjective: Observed Behavior – Eager to come out of carrier but was hissing during any medical handling and urinated during exam. Was sedated for radiographs and medical procedures Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = ~50bpm BCS 8/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: adult dentition, oligodontia, moderate dental tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs have increased harsh BV sounds bilaterally ABD: Non painful, no masses palpated, distended and unable to deeply palpate U/G: FS, spay scar noted when shaved MSI: Ambulatory x 4, skin free of parasites, no masses noted, haircoat has ash and smells of smoke CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Unkempt haircoat with ash and smells of smoke Increased harsh lung sounds bilaterally Plan: CTM while at BACC 2 view CXR-nsf, no lung changes Fluorescein stain ou-negative Rec weight loss Recheck breathing tomorrow Prognosis: good SURGERY: spayed
