Medical Summary DVM Intake Exam Findings History Subjective Observed Behavior – shy Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective P =160 R = wnl BCS 5/9 EENT: black mass on margin of right eye, small. evidence of ear mites and self mutilation of ears Oral Exam: oligodontia, severe gum disease of remaining teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment ear mites mass on eyelid dental disease Plan ivermectin 0.15ml sq need dental eventually mass probably benign but rexx biopsy
