EENT: Eyes clear, ears clean, blood observed at both nostrils (right > left)
Oral Exam: mandibular jaw fracture (displaced to the right — motion at TMJ unimpeded
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Intact female (wnl)
MSI: Weak hindlimbs, no palpable fractures, patchy areas of hairloss on ears, nose, and all four feet
CNS: Mentation appropriate — decreased tail tone and hindlimb weakness
Assessment: Traumatic injuries to jaw (mandibular fracture) and sacral nerves (tail-pull injury). Underweight with suspect dermatophytosis (Wood’s lamp negative).
Prognosis: guarded to fair
Plan: Provided 0.08 ml simbadol SQ, 25 ml SQ LRS +/5% dextrose, nutrical PO, and lyme sulphur dip at time of intake.
Continuing supportive care while at BACC. Setting up 0.08 ml simbadol SQ q24 for 5 days. Lyme sulphur dips every third day. Continued monitoring of overall condition and ability to eat. New Hope to find placement. If none, recommend humane euthanasia.

Follow up care at your regular veterinarian is recommended to ensure continued treatment. Your veterinarian will advise you if surgical sterilization is appropriate.
