Medical Summary DVM Intake Exam Estimated age: 4 years Microchip noted on Intake? Yes History : Owner could not keep pet in household, inherited from deceased father. Subjective: Curious, slightly anxious from being in kennel. Able to remove easily. Observed Behavior – BAR, became irritated and slightly fractious while retraining/scruffing for blood draw. Will let him settle down and pull blood for combo test tomorrow. Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T = nt P = 120 R = 20 BCS: 6-7/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Negative oral exam PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male castrated MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment In good health, anxious from restraint/being crated Plan: Rabies SQ right hind FVRCP SQ left hind Did not pull blood for combo test as became agitated during restraint Needs pyrantel mixed with food Prognosis: SURGERY: Already neutered
