Medical Summary DVM Intake Exam Estimated age: 9 wk Microchip noted on Intake? History : Stray – wandering with 2 siblings (Song and Whisper) Subjective: QARH Observed Behavior – Will try to flea. Bit someone yesterday. But was able to handle and do exam in towel. Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective T = P = R = BCS= 5/9 EENT: MPOD. MPND. Ears – mild brown d/c. Oral Exam: no ulcers. Kitten teeth. PLN: No enlargements noted H/L: NSR, NMA, MM pk, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Assessment URI Plan Doxy liq 0.15 cc po sid x 10 days. Monitor eyes – treat if worsens. Monitor eating. Prognosis: Good – fair (if unable to treat). SURGERY: Temporary waiver due to underweight and URI

Some yellow brown soft stool/ diarrhea in cage. Observed Behavior – Growls. Charges at front of cage. Will flee. Will try to bite. Needs towel for restraint. Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective T = P = R = eup BCS= 5/9 EENT: Eyes- OS mild MPOD but minimal chemosis and conjunctival hyperemia, OD clr. Ears – mild brown discharge. No nasal discharge noted. Oral Exam: no ulcers, full baby teeth. PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated. U/G: Female. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not done. A little dry feces noted on tail. Assessment: 8 week female DSH Mild conjunctivitis OS Plan: Unable to clean eyes and ears due to behavior. No eye tx for now – recheck in 2 days. Parvo test for panleuk – negative. Prognosis: Good SURGERY: Temporary waiver due to underweight.

OD clr. Ears – mod brown discharge. No nasal discharge noted. Oral Exam: no ulcers, full baby teeth. PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated. U/G: Female. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Assessment: 8 week female DSH Conjunctivitis OS Plan: Clean eyes and ears. Erythromycin OS BID x 7 days. Prognosis: Good SURGERY: Temporary waiver due to underweight.
