Vitani came in to the ACC with a large wound behind his left ear. Please consider helping Vitani today. He’s a real sweetheart and needs a foster or adopter by noon tomorrow.

Hello, my name is Vitani. My animal id is #17338. I am a male black cat at the Brooklyn Animal Care Center. The shelter thinks I am about 2 years old. – P

I came into the shelter as a stray on 30-Dec-2017.

Vitani is at risk for medical concerns. Vitani has a wound and abscess behind his left ear. While these wounds are recovering, recovery in the shelter environment with a lowered immune system can lead to illness.

[LVT Intake Exam] Microchip Scan: negative Evidence of Cruelty: no Observed Behavior: relaxed, allowed handling Sex: male intact Estimated Age: 2 years Subjective: describe how he animal is presenting and any known history Eyes: clear Ears:right ear is clean, but left ear- hematoma at around the base of the left ear Oral Exam: teeth -mild tartar and stainedno ulcer Heart: wnl Lungs: wnl Abdomen: no palpated mass Musculoskeletal: bcs- 5/9, no fleas ambx4 Mentation: BARH, no abnormal neuro sign others- called on call dr 1167, as per recommendation of Dr 0.9cc amoxiclav 62.5mg/ml given at intake. Preliminary Assessment: ear hematoma vs abscess Plan: needs dvm exam

DVM Intake Exam Estimated age: 1-2yrs Microchip noted on Intake? no History: Brought in as stray. Found in backyard with wound behind left ear. Subjective: Cat is Q/BARH Observed Behavior – Sideways at back of cage. Shy/timid. Little restraint required for exam. Shows some discomfort when left side of head touched (gently picks up paw and pushes away hand), but otherwise allows all body parts to be touched. Startles at quick movements and noises, but recovers quickly. Evidence of Cruelty seen -no Evidence of Trauma seen – yes Objective T = NA P = 150 R = wnl BCS 4-5/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; there is a large swelling behind the left ear with alopecia and scabbing Oral Exam: Healthy adult dentition; Grade 0/4 dental dz PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Bladder full, but soft; Intact male, both testicles palpable scrotally MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Abscess (caudal to left ear) Prognosis: Good Plan: Gave 0.55mLs Simbadol SQ, continue SID for additional 2 days. Sedated with 0.15cc Telazol IM. Applied artificial tears OU. Clipped area and scrubbed with chlorhexidine. Used 10 blade to puncture center of swelling – large amount of blood tinged purulent debris. Widened opening with sterile hemostat. Flushed with dilute chlorhexidine. Gave 0.4mLs Onsior SQ. Continue Clavamox drops – 0.9 mLs PO q12h x 10d. Keep in medical for monitoring. Consider drain placement at time of neuter if ear has re-abscessed. SURGERY: Okay for in-house surgery

2/01/2018

Progress exam History: Intake 12/31-abscess caudal to left ear. Was sedated at intake and abscess was lanced and flushed. Started on clavamox, simbadol (until 1/2), and given one dose of onsior. Subjective: BARH. Very nervous but allowed all handling. No c/s/v/d. Abscess is still open with very small amount of serous d/c. No nasal or ocular d/c. Good appetite. FS 5/7 bm in litter box. Does not seem painful. Objective P = wnl R = wnl BCS 5/9 EENT: Eyes clear, ears clean with draining abscess behind left ear with thick scab and mild serous d/c, no swelling around area, no nasal or ocular discharge noted Oral Exam: adult dentition, no oral lesions PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat (see above for abscess) CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: Draining abscess (caudal to left ear) Prognosis: Good Plan: Keep in medical for monitoring Continue clavamox until 1/10 No pain medications indicated at this time.

3/01/2018

Recheck abscess behind left ear S/O: BARH. Active, attention seeking, allows all handling. EENT: Eyes clear, no ocular or nasal discharge HL: No sneezing, normal thoracic auscultation INTEG: Full healthy coat. Wound at caudal base of left ear, tissue over center of wound thickened, appears necrotic. No swelling or discharge. MS: Ambulatory x 4 UG: Male A: Resolving abscess at base of left ear P: Skin over wound likely to slough off but expect it to heal well by second intention. Continue to monitor while at BACC. Good prognosis

KNOWN HISTORY:: Vitani was brought in as a stray, so we cannot speak to his behavior in his previous home.

MEDICAL BEHAVIOR:: Active, attention seeking, allows all handling.

Cage Condition:: Cage is recently cleaned

Reaction to assessor:: Vitani was relaxed and lying down on his blanket near the front of the kennel.

Reaction when softly spoken to:: Vitani looks over at the assessor with soft eyes, then rolls around while kneading in the air.

Reaction to cage door opening:: Vitani tenses up and lip licks.

Reaction to touch:: Vitani seems hesitant at first but slowly starts to warm up with attention. He perks up when pet, rolls around and leans in for rubs on his cheeks.

Reaction to being picked up:: Vitani was a bit tense when picked up, but remains calm and allows all handling.

VOCAL:: Quiet

CHARACTER TYPE: : Shy ,Sweet,Affectionate

Behavior Asilomar: H – Healthy

BEHAVIOR SUMMARY:: Vitani interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.
