Medical Summary DVM Intake Exam Estimated age: 2-3 Microchip noted on Intake? n History : Hit by car per police brought in panting and hyperreflexive, uncoordinated, intention tremors–would try to bite readily sedated with telazol IM 0.1ml. notable reaction: for about 5 minutes was in deep plane of anesthesia with eyes rolled back, but had completed worn off in 20 minutes. Subjective: sedated for radiographs/whole body–no breaks seen. no obvious soft tissue injuries either Observed Behavior – see above Evidence of Cruelty seen – n Evidence of Trauma seen – yes Objective P = 200 R = panting > 120 BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: wnl PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, panting ABD: Non painful, no masses palpated U/G: intact male MSI: flea dirt, can ambulate but with jerky movements, no masses noted, healthy hair coat CNS: hyperflexia, tremors Assessment: hit by car, possible brain injury Plan gave 0.5 ml simbadol, 0.25 ml meloxicam (5mg/ml) Prognosis: guarded/poor
