Subjective: RHL tib/fib traumatic amputation, old and healed
Observed Behavior – BAR
Evidence of Cruelty seen – n
Evidence of Trauma seen – y
Objective
P = 160 R = wnl BCS 5/9
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: decid teeth
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: male
MSI: Ambulatory x 3 , fleas
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: rh foot amp
Plan: no treatment
Prognosis: good
SURGERY:
Temporary waiver due to age
