Medical Summary 01/24/16 10:36 INITIAL EXAM approx 2 month old male intact DSH kitten left eye shows chronic herpes eye infection, globe likely ruptured previously, eye is shrunken and scarred mild nasal discharge – URI heart and lungs WNL on auscultation male intact 2 descended testicles full hair coat – no fleas seen ears have moderate waxy debris, no ear mites BAR, eating well, hydrated plan: gave FVRCP vaccine SQ over right front leg sign up for deworming with panacur and ponazuril start doxy 0.25 ml PO SID x 10 days start erythromycin OU BID x 10 days needs new hope placement
